Chiari Malformation: Inside and Outside the Dura – Clinical and Pathological Perspective on Intra- and Extradural Pathologies

Dr. Petra Klinge’s lecture presents Chiari malformation as a disorder involving abnormal anchoring and altered biomechanics, rather than solely a posterior fossa condition. She discusses extradural myodural bridges that connect the suboccipital muscles to the dura and help regulate CSF movement, highlighting evidence of altered collagen structure and impaired mechanical function, particularly in patients with connective tissue disorders such as EDS. Ultrasound and biomechanical studies are presented as emerging methods for evaluating these structures and their relationship to Chiari symptoms.
The lecture also examines intradural pathology, including tonsillar scarring, gliosis, tethering, and PICA vascular abnormalities, and their potential effects on cerebellar blood flow, cognition, and surgical outcomes. Preliminary findings suggest that Chiari decompression may improve cognitive function and neurological outcomes, particularly in younger patients, and that certain vascular and tonsillar abnormalities may be associated with better postoperative outcomes. Overall, the lecture emphasizes the importance of considering anchoring, vascular function, CSF dynamics, and cerebellar health in understanding and treating Chiari malformation.
