Chiari in the Youngest Patients: The Challenge of Atypical Presentations and Complex Management

Dr. Konstantina Svokos provides an in-depth overview of Chiari malformations in infants and young children, highlighting their complex presentation, associated conditions, and nuanced management. It begins with historical background and diagnostic fundamentals, including MRI findings, classifications from Chiari 0 through Chiari IV, and the role of CSF flow studies. The presentation explains how symptoms arise from crowding at the craniocervical junction, why tonsillar descent does not reliably correlate with severity, and how Chiari may be linked to conditions such as hydrocephalus, syringomyelia, craniofacial disorders, connective tissue disease, and tethered cord. Clinical features—from headaches and sleep apnea to scoliosis and brainstem dysfunction—are reviewed alongside criteria for surgery. The talk concludes with a detailed look at surgical decompression techniques, intraoperative considerations, and goals of restoring CSF flow, reducing syrinx progression, and protecting neurologic function in the youngest patients.
